Information for New Patients

Our services are available for patients who are homeless or vulnerably housed in the Exeter & Barnstaple areas in Devon.

Registration is easy and patients only have to fill in a few paper forms before being registered.

 

Forms can be collected at the reception desk.

 

Some patients find reading and writing difficult. It is not a problem and there will always be someone  to help. Our outreach 

nurses will also contact patients and register rough sleepers with the surgery.
